Employer Reimbursement Plans

  • OSP offers an Individual Coverage Health Reimbursement Arrangement (ICHRA), which is a type of employer-sponsored health insurance plan.

    Full-time employees are eligible for medical benefits after working for at least 90 days and averaging 30+ hours per week.

    The company provides employees with a monthly allowance or reimbursement to help cover the cost of individual health insurance plans they purchase independently. These plans can be tailored to meet an individual's healthcare needs and preferences. Employees can choose the insurance plan that best suits them, and the company reimburses them up to a certain amount, which is determined by their age. ICHRA offers flexibility, as it allows employees to select their preferred insurance plan and coverage, and it can be particularly beneficial for those who may not have access to traditional group health insurance.

    Notice of Availability of Form 1095-C

    Old Scratch Pizza prepares Form 1095-C, Employer-Provided Health Insurance Offer and Coverage, each year in accordance with the Affordable Care Act (ACA).

    Beginning with the most recent tax year, copies of Form 1095-C will no longer be automatically mailed to employees. However, employees may request a copy at any time, free of charge.

    If you would like to receive a copy of your Form 1095-C, please contact shayna@oldscratchpizza.com.

    Once requested, a copy will be provided within 30 days of the request.
